The question of the artist is sort of interesting. It shows how book covers get done in the publishing world: Whenever a St Martin's Press book gets nominated for cover art (there's an Anthony Award for best cover art, for example) the nominee is always David Rotstein. That's because David is the Art Director. But he didn't do the picture; he set out what he wanted, but he didn't draw it. The actual cover is usually drawn by an artist who's hired for the job. In my case it was a gentleman named Stefano Vitale.
If you look on the inside back flap of the US edition, you'll see four names:

Yes, the ARC is always a paperback, and it's printed before the cover is finalized, so you get grotty white and a system font. Also, the copy on the ARC is written by a random marketing person, whereas the copy on the final flap was written by me (I hope...I haven't seen the final inner flap myself yet).

Kudos to whoever is behind the cover art! I love how bright the colors are (mostly because it makes it easier to find on store shelves).
